For discharge planners, hospice & home health, elder law, and care managers
Refer a family — and actually hear how it turned out.
I'm Amy. I place families across senior living — independent living, assisted living, and memory care — throughout North Metro Atlanta: East Cobb, Roswell, Marietta, Alpharetta, Johns Creek, Sandy Springs, Dunwoody. When you send a family my way, two things happen that usually don't: I reach out to them the same day, and I come back and tell you how it turned out.
What you can count on
I contact your family the same day.
You're often working against a discharge clock. I move on your timeline, not mine.
You'll hear how it ended.
Most referrals vanish into a black hole — you never find out whether the family got placed, where, or whether they're okay. I close that loop myself. When the family you sent is settled, and with their okay, I'll let you know how it went.
Placement is the entire job.
I'm not a national call center buying and reselling leads. I'm one person, in your area, who places families into senior living communities — independent, assisted, and memory care, where I go deepest. Whatever the acuity of the case you're holding, I won't embarrass you in front of the family who trusted you.
I only recommend places I've walked.
Every community I'd put your family in front of, I've toured, met the director at, and seen on an ordinary weekday afternoon when nobody's performing for a visitor. If I haven't been inside, I won't send anyone there.
It runs both ways.
When a family I'm helping needs hospice, home health, an elder law attorney, or a care manager, I send them to the people I trust locally. Refer to me and you're not feeding a one-way pipe — you're part of a two-way network.
One ask before you reach out
Please leave protected health information out of your email or text. No patient names, diagnoses, or medical details in writing. Just tell me who you are, how to reach you, and how soon — I'll call the same day and we'll talk the family through by phone. It's simpler for you, and it keeps everyone protected.
Send a referral
No intake form. Call, text, or email me directly — the same way you'd reach any colleague you work with regularly.
Call or text (404) 941-4667 — that's my number, not a front desk.

Tell me who you are, how to reach you, and how soon the family needs help. I read every referral myself and get back to you the same day.
For the family you refer: my services are free to them — a community pays my fee only if they choose to move in, and they can stop working with me at any time. I disclose this up front on every intake, in line with Georgia's senior-living referral disclosure requirements. How I work →
